Once you have completed your Pack 95 registration above please locate your BSA Health Forms and Publicity Waiver located in your orientation packet, fill them out, and submit them.

A completed BSA health form is required for all adults and children and must be submitted to the Pack Secretary each year. Additionally the BSA publicity waiver must be filled out for each scout in the program and also submitted to the Pack Secretary each year. If you have lost the provided paper copies you can download and print additional copies at the links below or contact the Pack Secretary directly via the email button below for additional assistance.
